A downloadable MOSAIC Strict card
Though designed mainly for a game set in a version of the modern world, this system-agnostic haunted location can be used in any tabletop role-playing game system.
You don't even have to download the PDF or the text file. The full text is in the images and below. The text is designed to be MOSAIC Strict.

The Beast's Home
MOSAIC STRICT LOCATION
Though the Beast 666 has not yet been born, their house exists. It travels from town to town, from city to city, disappearing from one place and appearing in another. The house's appearance and floorplan adjust to match its new location, though it always looks abandoned. Anyone who enters is locked in, and unless they are the Beast 666, they are trapped there until they starve or the house moves on, whereupon any survivors are transformed into cursed dolls.
Player Characters might be simply curious, or maybe investigating a disappearance. Once locked inside, every 10-30 (d3x10) minutes the house does something, or something happens. The events are on the back of this card, in the order they occur.
To escape, the PCs must find the hidden room where the dolls are stored, and destroy them all. The house, horrified, will eject every living human in the house and prematurely move to its next destination. The house can only be defeated, not destroyed, until the Beast 666 is born.
Happenings in the Beast's House
This could also be used as a random table but the characters will likely only face the house once, so going in order is better.
- The layout of the house changes.
- All lights carried by the PCs cease to work
- Dark whispering figures caper on the edge of vision
- Dolls make crying sounds in different rooms, trying to split the group
- The walls bleed, blood burns like acid when touched
- Rotten boards give way under a PC's feet
- Animated dolls throw objects at the PCs from a distance
- One of the floorboards begins screaming and sobbing when stepped on
- Poltergeist attack: The furniture itself batters the PCs
- Other PCs appear to each other as rotting corpses / walking dead
- Screaming from multiple distant rooms (attempt to split the group)
- Starving children attack with cannibal intent
- Character looking into a mirror notices a menacing figure that only appears in the mirror
- Doppelgangers of the PCs attempt to infiltrate the group in the dark
- A feast is laid out, but the food is actually rotten
- Swarmed by animated dolls (particularly any separated PCs)
- Wallpaper peels, revealing pulsing flesh as the walls close in
- Waves of rusted knives thrown at the PCs from all directions (animated dolls or poltergeist effect)
- Almost solid swarm of hungry roaches, waist high
- Time dilates, a day outside the house is week inside it or vice-versa, either way starvation is accelerated
